Multimedia Request Form
Faculty use this form to request processing of narrated slide presentations, or help with adding audio and video files to course sites.
Faculty submitting files for processing will be directed to a second page where they can upload and send those files.
Online Course Development Contract Request Form
Program directors can request the contract for the development of a new online course.
Online Course Review Request Form
Faculty and program directors can request reviews of online courses.
Instructional Designers will be in contact with the faculty during the review process.
Content Import Request Form
Faculty or Program Directors who would like the IRC to import content from one course site into another should use this form.
Unofficial Sakai Participant Form
Sakai users who do NOT have a JHED ID (i.e. guest lecturers and presenters) must be added to Sakai by a system administrator. Please use this form to request access for unofficial participants.
Face-to-Face Audio/Video Equipment Request Form (opens in new window)
Faculty should complete and submit this form to request audio and/or video equipment for face-to-face class needs. Once submitted, you request will be emailed to the proper Faculty Services Coordinator at your physical teaching location. Note: this form is NOT for online instructors.
Desk Copy Request Form (opens in new window)
Please note that a desk copy is provided by a publisher for texts that are adopted and ordered for a course. To review a text, please contact the publisher directly. Allow extra time to receive the copy before the semester begins. Contact Megan McGuire (410.516.4578) with any questions.
